Print and Fill out application – the application can be downloaded by clicking here.

The application can be submitted to the Main Office with the following forms:
- Application/Registration Fee

- Immunization form

- Award Letter (if applicable)

- USDA Form

Parents will be notified of Admission status by letter or phone call.
